Oak Haven Community Care Center is
a skilled nursing and rehabilitation center located in Avoyelles
Parish, owned and operated by CommCare Corporation.
Our staff specializes in providing
skilled nursing, long term care, and short term rehabilitation such
as physical, speech, occupational and restorative therapies. We
serve Center Point and the surrounding areas. Our newly
remodeled facility works closely with other regional medical
facilities, including HealthSouth, Avoyelles Hospital (Marksville),
Rapides Regional Medical Center, St. Francis Cabrini (Alexandria),
Huey P. Long Hospital, V.A. Hospital (Pineville) and Bunkie General
Hospital.
